This is a styandard SQL Server input page it should contain TXT boxes of the fields required that is : Country,County,Hub,GolfClub,Protopage Site, PostCode,What3words,E'Mail Address, Phone Number
Note I have extended the requirements of the SQL Server to those announcesd at My Study Buddies.
